Preventing admission to residential care by using existing CCBC properties as temporary accommodation has made it possible to resettle complex individuals into appropriate housing.
Working with Housing Services, a process was developed to ensure effective use of adapted properties across North Wales. This initially evolved into the Adapted Property Matching Register which has now changed again to become SARTH (Single Access Route to Housing) and Housing Solutions.
What’s changed?
Without Canolfan Marl apartments being available and joint working with Disability Services and Housing agencies, the prospective tenants may have had to be placed temporarily in residential or nursing care or in less suitable temporary accommodation which would have impacted on their independence and well-being.
This was achieved by SW and OT practitioners identifying whether the best temporary housing solution would be for someone to move into the apartments. A Multidisciplinary team panel would process the applications and monitor the tenancies.
Canolfan Marl apartments in Llandudno Junction were potentially suitable for all disabled people of Conwy.
What difference has it made?
12 service users were able to access and stay at the 6 Canolfan Marl apartments in 14/15. This prevented admission to residential or nursing home placement while suitably adapted accommodation was sourced or while adaptations were being carried out on their own properties and enabled the customers to remain close to their community support and families.
Case Study
Ms X has Multiple Sclerosis and was living in a 2nd floor flat. She experienced a relapse of her condition and was admitted to hospital and was not able to return home because she wouldn’t have been able to manage the steps. Following a period of rehab in hospital, Ms X was discharged to Canolfan Marl apartments. A referral to the Adapted Property Matching Services was completed and a suitable property was identified by Housing Services and following an OT assessment, it will be adapted for her needs now as a wheelchair user. Ms X has been living at Canolfan Marl for 3 months and hopes to be able to move within the next 6 months.
