- We will roll out a new Pathway to Care Approach to assessment within the context of the new assessment tool and framework as agreed with WG. This innovative approach should enable effective consideration of the outcomes required for people and assess the success of applying eligibility only after the point of re-ablement.

- Work with BCUHB to gain a clear understand of unscheduled admissions to District General Hospitals
- Work with BCUHB to develop preventative services
- We will implement the findings of our access to services project and implement the recommendations to ensure a proportionate response is received and that where people can be signposted to alternative non statutory services this is done.
- We will implement a pathway of care with Health partners that takes account of patient journey from hospital to Enhanced Health Care, Intermediate Care, re-ablement and to no services or 3rd sector support.
- To support people with Learning Disability we will Introduce a new assessment tool that focuses on people’ s ability not disability

- Develop a mechanism to record Service User outcomes and ensure adherence
- Development of the Children with Disabilities (CWD) Network, bringing together Health, Education and Social Services to create a single register of Children with Disabilities and from this develop a communication strategy utilising social media
- To analyse and recommend how the number of accreditation options for 16/17 year olds can be increased
All staff have aims that are linked to business plans and organisational priorities
Organisations often talk about the idea of the “golden thread” that links individual aims to team plans, to higher up strategic plans. If it’s done properly, it means that every member of staff is working towards a common goal within their team, and that the team helps achieve something as part of a larger strategy which may involve other agencies.
Communication of these ideas is central to making it work within a large organisation, so that everyone has a sense of “going in the same direction”. In Conwy, we’ve been developing ways of nurturing this idea within the staff group so that people feel empowered and included. [Read more…]
Planning, management and delivery of services is supported with reliable and timely information
To make sure that we are continually improving, we use information about how the services are performing and things we learn from listening to our “customers”. We analyse this information regularly and use it to make decisions about any changes that need to be made. This is a continuous process. Feedback days for staff keep them informed about findings from our audits, and any trends in performance that we need to be aware of, plus any findings from consultation with service users. A Quality Assurance Framework is in place which informs organisational learning and development
We take the quality of our work very seriously. It’s not just about how much we do, we want to know how well we did it. Through a detailed series of checks and audits, we methodically and routinely test various aspects of our work and then set about improving the service according to what we found.
This year, in addition to the regular audits we carry out, there have been a number of extra projects such as the “Engaging Fathers” work, and a specific piece of work looking at the referrals that come in to the Children and Families department. [Read more…]
