Mae trefniadau hefyd yn eu lle i sicrhau bod perthnasau gwaith da yn cael eu sefydlu gyda grwpiau eraill fel y Clystyrau Meddygon Teulu i sicrhau bod gwybodaeth yn cael ei rhannu ar draws pob sector yn Sir Conwy.<\/p>\n<p>Cytunwyd ar y Cylch Gorchwyl ar gyfer y gr\u0175p ac mae&#8217;n cynnwys cynrychiolaeth dinasyddion er mwyn sicrhau didwylledd, tryloywder ac atebolrwydd.
