Mae Partneriaeth Pobl Conwy yn bartneriaeth wedi\u2019i hadlinio yn sgil uno Partneriaethau Partneriaeth Plant a Phobl Ifanc a Cydbartneriaeth Ardal Conwy yn seiliedig ar ganllawiau gan gynllun ‘Cydamcanu – Cydymdrechu\u2019 Llywodraeth Cymru ar integreiddio partneriaethau a chynlluniau.<\/p>\n
Cynhaliwyd cyfarfod cyntaf Bwrdd Partneriaeth Pobl Conwy ym mis Ebrill 2015, mae’r bwrdd yn cynnwys partneriaid allweddol traws-sector strategol aml-asiantaeth i weithio ar egwyddorion a blaenoriaethau a rennir. Mae gwaith y Bartneriaeth yn hysbysu ‘Cynllun Integredig Sengl Un Conwy – Gweithio Gyda’n Gilydd am Ddyfodol Gwell\u2019. Cynorthwyo i gyflenwi\u2019r Grwpiau Deilliant Conwy (COGs) a sefydlwyd nifer o grwpiau tasg a gorffen.
