Yn hytrach na bod y person ifanc yn symud allan yn 18 oed, gallant bellach aros mewn Gofal Maeth nes eu bod yn 21, neu hyd at 25 oed os ydynt mewn addysg amser llawn. Mae&#8217;r cynllun Pan Fydda i\u2019n Barod hefyd yn ymestyn dyletswydd newydd i&#8217;r bobl ifanc hynny sy&#8217;n byw gyda Gofalwyr Maeth o Fewn y Teulu ac Unigolion Cysylltiedig, sy\u2019n gadarnhaol iawn.<\/p>\n<p style=\"text-align: left;\"><a href=\"https:\/\/conwysocialservicesannualreport.org.uk\/wp-content\/uploads\/2016\/09\/WIR-CYM.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-1321 alignright\" src=\"https:\/\/conwysocialservicesannualreport.org.uk\/wp-content\/uploads\/2016\/09\/WIR-CYM.png\" alt=\"wir-cym\" width=\"193\" height=\"424\" srcset=\"https:\/\/conwysocialservicesannualreport.org.uk\/wp-content\/uploads\/2016\/09\/WIR-CYM.png 193w, https:\/\/conwysocialservicesannualreport.org.uk\/wp-content\/uploads\/2016\/09\/WIR-CYM-137x300.png 137w\" sizes=\"auto, (max-width: 193px) 100vw, 193px\" \/><\/a>Rydym wedi cyfrannu&#8217;n ymarferol at greu polisi rhanbarthol ar draws y chwe awdurdod lleol yng Ngogledd Cymru.<\/p>\n<ul>\n<li>Cr\u00ebwyd taflenni gwybodaeth ar gyfer pobl ifanc, Gofalwyr Maeth a brodyr a chwiorydd.<\/li>\n<\/ul>\n<ul>\n<li>Darparwyd hyfforddiant i staff allweddol sy&#8217;n ymwneud \u00e2 \u201cPan Fydda i\u2019n Barod\u201d. Darparwyd sesiynau hyfforddi\u2019r hyfforddwr hefyd er mwyn cyrraedd y gweithlu ehangach.<\/li>\n<\/ul>\n<ul>\n<li>Er mwyn ein helpu i ddiwallu anghenion o ran \u201cPan fydda i\u2019n Barod\u201d, datblygwyd protocolau newydd ar y cyd \u00e2 Budd-dal Tai a hawliau Lles i sicrhau bod Gofalwyr Maeth a phobl ifanc yn ymwybodol o&#8217;u hawliau ac nad ydynt o dan anfantais yn ariannol.<\/li>\n<\/ul>\n<ul>\n<li>Cafwyd ymagwedd ranbarthol tuag at ymgysylltu gyda darparwyr annibynnol, yn enwedig o ran yr angen iddynt drosglwyddo i&#8217;r Awdurdod Lleol os ydynt yn dod yn Ofalwyr Maeth \u201cPan Fydda i\u2019n Barod\u201d.<\/li>\n<\/ul>\n<ul>\n<li>Anfonwyd gwybodaeth am &#8220;Pan Fydda i\u2019n Barod\u201d at bob person ifanc (16+) trwy&#8217;r newyddlen a ddatblygwyd gan y t\u00eem Ymgynghorwyr personol, sy&#8217;n targedu pob person ifanc (16+) sydd \u00e2 hawl i gael gwasanaeth gadael gofal.<\/li>\n<\/ul>\n<p>Wrth edrych ymlaen o ran y galw tebygol, dros y 12 mis nesaf, mae gennym hyd at chwech o bobl ifanc sy&#8217;n debygol o fod angen parhau yn eu lleoliad o dan drefniant &#8220;Pan Fydda i\u2019n Barod&#8221;, a deg yn rhagor o bobl ifanc y flwyddyn ganlynol.<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Mae\u2019r Ddeddf Gwasanaethau Cymdeithasol a Lles yn cynnwys dyletswydd newydd i gynnig y cyfle i unrhyw Blentyn sy\u2019n Derbyn Gofal Maeth aros yn eu hamgylchedd teuluol sefydlog, lle maent yn cael eu meithrin nes eu bod yn barod i symud ymlaen.
