Ers mis Awst 2015 mae\u2019r gwasanaeth gwaith cymdeithasol yn Ysbyty Gwynedd wedi cael ei leoli mewn swyddfa bwrpasol sy\u2019n canolbwyntio ar integreiddio rhwng y Gwasanaethau Cymdeithasol, PBC a’r Trydydd Sector.<\/p>\n
Mae holl adrannau Gwasanaethau Cymdeithasol y Gogledd Orllewin, Conwy, Gwynedd ac Ynys M\u00f4n wedi eu lleoli yma yn ychwanegol at Nyrsys Cyswllt Rhyddhau, Rheolwyr Gwl\u00e2u, cynrychiolwyr y Trydydd Sector a’r t\u00eem gweinyddol.<\/p>\n
Yr amcan cyffredinol oedd gwella cyfathrebu ymhlith yr asiantaethau i hwyluso rhyddhau diogel ac amserol o Ysbyty Gwynedd.<\/p>\n
Cynhelir cyfarfodydd y Bwrdd yn ddyddiol i nodi cleifion sy’n barod ar gyfer cynllunio i\u2019w rhyddhau ac i amlygu unrhyw faterion posibl a allai achosi oedi cyn rhyddhau.<\/p>\n
Mae cyfarfodydd Oedi wrth Drosglwyddo Gofal wedi cael eu sefydlu sy’n cael eu cynnal bob prynhawn Gwener i nodi cleifion sy\u2019n barod i’w rhyddhau ond gall materion nad ydynt yn feddygol achosi oedi. Mae’r fforwm hwn yn ystyried ffyrdd eraill o ddiwallu anghenion y cleifion os nad yw\u2019r hyn a nodwyd ar gael.<\/p>\n
