Mae Gofal Cymdeithasol Cyngor Bwrdeistref Sirol Conwy wedi bod yn brysur yn datblygu Canolfannau Iechyd a Lles sy\u2019n cynnig gweithgareddau Iechyd a Lles ar draws pum ardal Conwy er mwyn cyrraedd holl gymunedau Conwy gyda’r nod o helpu pobl i aros yn iach ac mor annibynnol ag y bo modd o fewn eu cymunedau eu hunain am gyhyd ag y bo modd, yn ogystal \u00e2 galluogi gwaith ardal integredig ar y cyd rhwng Gofal Cymdeithasol ac Iechyd.<\/p>\n
