{"id":1269,"date":"2016-09-20T11:47:46","date_gmt":"2016-09-20T10:47:46","guid":{"rendered":"https:\/\/conwysocialservicesannualreport.org.uk\/?p=1269"},"modified":"2016-09-20T11:47:46","modified_gmt":"2016-09-20T10:47:46","slug":"gweithgareddau-cymunedol-a-lles-ar-gyfer-rhai-dros-55-oed","status":"publish","type":"post","link":"https:\/\/conwysocialservicesannualreport.org.uk\/cy\/gweithgareddau-cymunedol-a-lles-ar-gyfer-rhai-dros-55-oed\/","title":{"rendered":"Gweithgareddau Cymunedol a Lles ar gyfer rhai dros 55 oed"},"content":{"rendered":"<p>Nod y gweithgareddau Cymunedol a Lles yw lleihau nifer yr atgyfeiriadau i wasanaethau statudol gan y bydd Lles Dinasyddion Conwy yn parhau i wella trwy nifer o wahanol ddulliau fel integreiddio cymunedol, addysg ar weithgareddau Lles ac ennyn hyder.<\/p>\n<p>Ym mis Tachwedd 2015, buom yn llwyddiannus wrth recriwtio unigolion o safon uchel a rhai sy&#8217;n canolbwyntio ar y gymuned drwy Gyllid Gofal Canolradd, sydd yn ei dro\u2019n cryfhau ein T\u00eem Lles Cymunedol, a arweinir gan Jayne Neal.<\/p>\n<p>Canolbwyntiwyd ar ddatblygu rhaglen haf, hydref, gwanwyn a gaeaf o weithgareddau lles yn y Ganolfan Lles ar ei newydd wedd yn Nh\u0177 Llywelyn, Llandudno. Mae gweithgareddau hefyd yn estyn allan i rannau eraill o\u2019r Fwrdeistref gan gynnwys Abergele, Llanfairfechan, Bae Colwyn a Llanrwst.<\/p>\n<p>Rydym yn ymgysylltu ag unigolion a sefydliadau i gyflwyno\u2019r sesiynau hyn ar draws y fwrdeistref, gan gadw&#8217;r gwariant lleol i Gonwy \u2013 rydym yn galw\u2019r rhain yn ein darparwyr. Ein nod yw parhau i weithio mewn cydweithrediad cryf gyda\u2019r darparwyr hyn i gynnal y rhaglen o weithgareddau Lles ar \u00f4l mis Mawrth 2017, sef pryd y daw\u2019r cyllid i ben.<\/p>\n<p>Mae&#8217;r prosiect hwn wedi ennill momentwm cryf ac wedi bod yn llwyddiant wrth estyn allan at unigolion gydag a heb anghenion wrth wella eu hiechyd personol a lles ar draws Conwy.
